《新英格兰医学杂志》于4月10日发表了来自埃默里大学医学院神经外科的一项开创性研究。这项多中心随机临床试验比较了急性颅内出血患者接受早期微创手术与传统治疗的康复效果。<br><br>研究纳入了300名急性脑出血患者,血肿体积介于30-80ml之间。参与者被随机分为手术组和非手术组,手术组在出血后24小时内接受了微创手术以清除血肿,而非手术组则接受了标准治疗。研究主要采用改良Rankin量表评分来评估疗效。<br><br>结果显示,手术组患者在180天后的改良Rankin量表评分平均为0.458,显著优于对照组的0.374,尤其是叶内出血患者获益更显著,而基底神经节出血患者改善不明显。<br><br>首席研究员Gustavo Pradilla博士表示:“这项研究将可能改变我们对脑出血引起的中风的治疗方法。手术的时机和技术是改善疗效的关键。”<br><br>此外,研究发现,与非手术组相比,手术组在中风后前30天的早期死亡率减少了近一半。早期微创手术通过小型切口清除颅内血块,这种及时精准的干预为患者的康复带来显著改善。<br><br>这些发现可能推动全球医学指南的更新,并为全球数千名受到严重中风影响的患者带来新的希望。
